Registration Forms
By learning more about each form below, choose the one that best suits your student! Remember: each student needs their own form! Click, print, complete, and return through email. Please include forms for all siblings within the same email. A payment link will be texted to you once your registration forms are received.
New Student Forms
This form is used for someone completely new to our program. Please include a copy of your child's baptism certificate with your form.
Returning Student Form
This form is used for students who are already attending our Faith Formation program and are entering grades Kindergarten, 1st, 3rd, 4th, 5th, 6th, or 7th. 2nd Grade and 8th Grade must use the Sacrament Preparation forms located below.
1st Holy Eucharist
This form is used for students preparing for the Sacraments of Holy Eucharist and Reconciliation. All 2nd grade students will use this form. Students coming from Catholic schools will also use this form.
Confirmation
This form is used for students preparing for the Sacrament of Confirmation. All 8th grade students will use this form. Students coming from Catholic schools will also use this form.
Parent Links
Parent Communication
We use GroupMe for all Faith Formation communication between parents, teachers, and the director. Parents are responsible for remaining informed.
